Abstract
RISIKO KEJADIAN ASFIKSIA NEONATORUM PADA PERSALINAN KALA II MEMANJANG, AIR KETUBAN BERCAMPUR MEKONIUM DAN USIA IBU Intan Kumalasariˡ*, Zizke Rusella2 1* Program Studi D3 Pengawasan Epidemiologi, Poltekkes Kemenkes Palembang, Indoensia 2Akademi Kebidanan Persada, Palembang-Indonsia Email: [email protected], [email protected] Abstract Asphyxia neonatorum still ranks second as a cause of death for newborns in the first week of life after Low Birth Weight in Indonesia. Failure to breathe spontaneously and regularly is the leading cause of death. Identifying risk factors is an effort to reduce the incidence and mortality rate of babies with asphyxia, including the second stage is prolonged, amniotic fluid mixed with meconium and maternal age. The purpose of this study was to determine the relationship between the second stage of elongation, mixed meconium amniotic fluid and maternal age together or in part with the incidence of asphyxia neonatorum. This study uses an analytical survey with a cross sectional approach. The population in this study were all newborns at Muhammadiyah Hospital Palembang who were recorded at the medical record installation, using a total sampling technique of 156 births in the period from 1 September to 1 October 2016. The data were univariate and bivariate analysis with chi-square test. The results showed that there was a significant relationship between prolonged second stage of labor and neonatal asphyxia (p=0.002) and OR=42.600, which means that labor with prolonged second stage of labor had a 42.6 times risk of causing neonatal asphyxia, as well as meconium-mixed amniotic fluid where p=0,000 and OR = 21,719 which means that the amniotic fluid mixed with meconium has a 21,719 times risk of causing asphyxia, but not for maternal age (p value 0,603; OR = 0,858) Early detection through routine antenatal care can minimize risk factors and the incidence of asphyxia neonatorum. Keywords: asphyxia neonatorum, maternal age, mixed amniotic fluid mixed with mekomenum, prolonged second stage of labor Abstrak Asfiksia neonatorum masih menempati urutan kedua sebagai penyebab kematian bayi baru lahir pada minggu pertama kehidupan setelah BBLR di Indonesia. Kegagalan bernapas secara spontan dan teratur menjadi penyebab utama kematian. Mengidentifikasi faktor risiko merupakan upaya memperkecil angka kejadian dan tingkat kefatalan bayi dengan asfiksia, diantaranya karena kala II memanjang, air ketuban bercampur mekoneum dan usia ibu. Tujuan penelitian ini untuk mengetahui hubungan kala II memanjang, air ketuban bercampur mekoneum dan usia ibu secara bersama maupun sebagian terhadap kejadian asfiksia neonatorum. Penelitian ini menggunakan survei analitik dengan pendekatan cross sectional. Populasi dalam penelitian adalah semua bayi baru lahir di Rumah Sakit Muhammadiyah Palembang yang tercatat di instalasi rekam medik, menggunakan teknik total sampling sebanyak 156 kelahiran pada periode 1 September sampai 1 Oktober 2016. Data di analisis secara univariat dan bivariat dengan uji chi-square. Hasil penelitian diperoleh hubungan bermakna antara Persalinan Kala II memanjang dengan Asfiksia Neonatorum (p=0,002) dan OR=42,600 yang berarti persalinan dengan Kala II memanjang berisiko 42,6 kali menyebabkan Asfiksia Neonatorum, begitu pula dengan air ketuban bercampur mekoneum dimana p=0,000 dan OR=21,719 yang bermakna air ketuban bercampur mekoneum berisiko 21,719 kali menyebabkan Asfiksia, tetapi tidak untuk usia ibu (p value 0,603; OR = 0,858). Deteksi dini melalui perawatan antenatal secara rutin dapat meminimalisir faktor risiko dan kejadian Asfiksia Neonatorum.
